One of the most important aspects of all healthcare is patient safety, never the less adverse events is one of the biggest problems. In Sweden adverse events was calculated to cost society 7-8 billion SEK in 2013 and you run a 10 times a high a risk dying from being treated in a hospital then in a traffic. Markörbaserad journalgranskning is an established and standardized method for recording statitics around adverse events. It’s an retrospective method where medical records are (manually) checked for specific triggers that indicate an high risk for adverse events. The identified records are then analysed to find quality problems that can be addressed in the organisation. In Sweden all hospitals are obliged by the government to preform 20-40 reviews every month.

The problem with this type of review is that it is retrospective and not a proactive support for addressing adverse events. It has a low resolution, is very cost inefficient and only checks a randomised number of medical records.

This session will show the radical effects of integrating the trigger based identification mechanism directly in the EHR. This will automatically provide a X-100% coverage of the medical records in the installation and a 360 degree view of the organisation. Triggers can be combined and priorities and changed in real time based on new local knowledge. Furthermore, a real time based trigger tool give the organisation the possibility to react on the triggers and prevent adverse events from happening. Since the methodology is already accepted and in use, implementation of a real time based version will be simple and meet very low resistance in the organisation.

By basing the solution on a structured medical record this kind of solutions can be achieved to a fraction of the cost compared with a natural language processing tools that scans free text medical records.
